Final Report
Final reports should be organized as outlined below:
- Project Title, Funding Dates and an Updated Project Abstract.
- Project Accomplishments: Describe how you met your program plan and objectives.
Be specific and cover timeline and methods used, and, where appropriate, use
quantifiable terms on numbers of programs or activities, where the programs
took place, and audiences/clientele served. If you have questions, contact
Peyton Smith, assistant vice chancellor for extended programs, 262-8214 or
plsmith@wisc.edu.
- Benefits to date: Describe how the intended audience(s) used the information
and benefited from your project.
- Fiscal commitments: Report matching funds, gifts, grants, sponsorships,
or program revenue from fees and other sources that have resulted from your
project.
- Project continuation: Describe ongoing activities that will result from
the efforts of your project (e.g., project continued through other funding
sources, new program initiatives, etc.)
- As a final condition of the grant, please go to the Wisconsin
Idea Submission Web site and provide specific information about how
and where your education, research, clinical or outreach programs significantly
involved Wisconsin citizens in tangible ways, and how that work is making
an impact. Please indicate on your final report that you have completed
the online database submission.
PLEASE NOTE: Final reports should be limited to five double-spaced
typed pages.
Final Report Deadline: February 1, 2008 for projects that
end December 31, 2007.
